Whether it happens in our own lives or that of a loved one, there may come a point at which living with a health condition, whether acute or chronic, can become extremely tiring – whether it's feeling the depths of depression for the first time, living with the uncertainty of a new traumatic brain injury, or simply aging in an environment that, while familiar and comforting, may no longer be conducive to your body’s needs. In these times, you might find yourself wondering how to find solace in your circumstances, how to find the specialized care you need, and how to ensure that you won’t feel alone in handling your health reality.
I started New Phase Navigation to provide comfort, direction, and preparation in the face of health transitions. We will all deal with aging at some point, and in some unfortunate circumstances, we might experience a dramatic illness or injury that will drastically change our lives.
In my years working with families who have lived through this at various stages, I know how confusing and overwhelming it can be to understand how to navigate our healthcare system to receive the services we deserve. Even access to basic needs can entail complex processes. No one should feel alone in navigating this. I believe our power comes from forming strong relationships with ourselves and our community.

Our Team
Zeina Toure, LCSW
Counselor, Case Manager
Zeina was born in Dakar, Senegal, but grew up in Northern Virginia, outside of Washington D.C. She was fortunate to have lived in an incredibly diverse part of the country. After studying Psychology and Human Development, Zeina her received her MSW at the University of Denver. She has been a social worker in a variety of settings for 10 years. She has worked with refugees settling into the US, adults with severe and persistent mental illness, individuals with new injuries, and in all of her experience, Zeina values the importance of compassion as fuel for fostering positive environments. Zeina has worked in inpatient and residential settings and enjoys finding small ways to create connection and fluidity in the systems she is a part of. She is passionate about learning and has supervised and mentored social workers earning their degrees. She is certified with the Accredited Case Management Association and is proud to be alongside you on your journey.

Christie Musselman, RN
Case Manager
Christie was born and raised in the Denver area. She lived and studied in Guadalajara, Mexico, and received a degree in Sociology and Spanish. Upon returning to Denver, she worked in social services in the metro homeless community before returning to school to get her Bachelor of Science in Nursing from the University of Colorado at Denver. She spent several years as a clinical RN in the Inpatient Acute Care setting before transitioning to Case Management. She has worked in Case Management for the past 8 years, and has been the Nurse Educator for a local Inpatient Care Management Department.
