Participate in a Phone or Video Consultation
You'll meet with one of our clinical case managers who will spend time ​getting to know you and your support systems, your health condition, recent challenges, and your overall goals. We'll see how our services can support you on your health journey and discuss a pricing structure for your tiers of service.

Get matched with your Navigation Team Case Manager to complete an Initial Assessment
You will be matched with your Navigation Team case manager, who will meet with you or your loved one to complete an on-site assessment. Your case manager will be leading your plan of care with the close collaboration of your other Navigation Team Members. At the initial assessment, we will get to know your more and learn about your living environment, supports, how to manage your ADLs, and discuss goals to support the activities and relationships that are important to you.

Access your Care Portal and start your journey with the support of your Navigation Team!
You'll receive access to your Navigation Care Portal (or printed Navigation Care Guide) designed help you maintain important contacts, set visits with your caregiver, counselor, and case manager, and keep your 90-day plan of care on track. With the ongoing support of your Navigation Team, you'll put your plan into practice, allowing you to feel more confident and prepared as you continue your journey
